Creatine Side Effects: What Daily Use Actually Does to Your Body Over Time

Something happens when you tell your doctor you take creatine. More often than not, they look up from your bloodwork, point at an elevated creatinine level, and tell you to stop immediately. The thing is — that number going up is often expected. And it usually doesn’t mean your kidneys are struggling.

That disconnect between what the research shows and what most people hear is why creatine is one of the most misunderstood supplements in the entire category. It’s also why so many people quit a supplement that, for a wide range of healthy adults, carries a genuinely solid safety record backed by decades of study.

Let me walk you through what the science actually says about creatine side effects — the ones worth watching for, the ones that are largely myth, and what your label is probably not telling you about quality.

The Real Creatine Side Effects (And Which Ones Are Exaggerated)

Creatine monohydrate has more peer-reviewed research behind it than almost any other sports supplement — well over 500 published studies at this point. Most of that evidence points in the same direction: at standard doses of 3–5 grams per day, it’s well-tolerated by most healthy adults.

That said, some side effects are real and worth knowing about.

Water retention. This is the most common one, and it’s not a myth. Creatine draws water into your muscle cells as part of how it works. Studies consistently show a 1–2 kg increase in total body water during the first week or two of use, particularly during a loading phase. For most people this levels off after the initial period, but some degree of intracellular water retention is part of what creatine does. If you have a competition weigh-in coming up, that timing matters.

GI discomfort. Stomach cramping, nausea, and loose stools are reported most frequently by people taking large single doses — specifically during loading phases where some protocols call for 20 grams per day divided into four servings. A 2008 review in the Journal of the International Society of Sports Nutrition found that splitting doses and taking creatine with food eliminated most GI complaints. Taking 10 grams at once on an empty stomach? That’s a recipe for a rough afternoon.

Muscle cramping. This one is genuinely complicated. Anecdotal reports are common, but controlled trials haven’t consistently reproduced it. A widely referenced review covering 14 separate studies found no meaningful difference in cramping rates between creatine users and placebo groups. The cramp complaints may have more to do with hydration habits — because creatine does shift water into muscle tissue, inadequate fluid intake during training can become a bigger issue than it would otherwise be.

The side effects that are not well-supported by evidence? Hair loss (the DHT-conversion concern traces back to a single 2009 rugby study that has never been replicated), liver toxicity in healthy individuals, and the pervasive idea that creatine somehow “stresses” your organs over time.

Why Your Doctor Might Be Telling You to Stop — And Why That Deserves a Second Look

Here’s the bloodwork issue, and it’s worth explaining properly.

Creatinine — not creatine — is a waste byproduct that your kidneys filter from your blood. It’s a standard kidney function marker. When you supplement with creatine, your body naturally produces more creatinine as a metabolic byproduct. This means creatine supplementation can raise your serum creatinine by roughly 10–15%, even in people with perfectly healthy kidneys.

A physician who doesn’t know you’re taking creatine sees that elevated number and flags it as a potential kidney problem. It’s a completely understandable clinical response. But it’s also why you should always disclose supplement use before bloodwork, and why a single elevated creatinine reading in an active person taking creatine isn’t the same as kidney disease.

The actual research on kidney function and long-term creatine use is reassuring for healthy people. A 2021 systematic review covering over 18 months of continuous supplementation in healthy athletes found no clinically meaningful changes in markers of kidney or liver function. Longer observational studies following recreational athletes on 5 grams per day have landed in the same place.

The important caveat — and I want to be direct here — is healthy individuals. People with pre-existing kidney disease, a single functioning kidney, or conditions like polycystic kidney disease are a different story entirely. If any kidney condition applies to you, this is a conversation to have with a nephrologist, not something to work around with a blog post.

The Quality Problem Nobody Mentions When They Talk About Creatine Side Effects

Here’s something that doesn’t get nearly enough attention: a significant portion of the GI issues, unexpected side effects, and general complaints about creatine may not be about creatine itself — they may be about what’s actually in your creatine.

Most creatine monohydrate sold in the US is synthesized chemically, which is fine in principle, but manufacturing quality varies considerably depending on the source. Some lower-grade creatine contains contaminants like dicyandiamide (DCD) and dihydrotriazine as byproducts of the synthesis process. These don’t appear on your label. They’re only detectable through analytical methods like HPLC.

Third-party certification programs like NSF Certified for Sport, Informed Sport, and USP Verified require batch testing for contaminants and verify that products contain what the label claims. But the majority of creatine products on the market have never been through that process. In supplement categories tested by organizations like ConsumerLab.com, it’s not unusual for 20–30% of products to fail purity or label-accuracy checks.

According to testing data from Qalitex Laboratories, a California-based ISO 17025-accredited testing facility, creatine products submitted for quality verification sometimes show creatine content that doesn’t match label claims by 5–15%. A product claiming 5 grams per serving might deliver 4.2 grams. That might not sound dramatic, but it compounds over weeks of use and makes it nearly impossible to accurately assess whether your dosing is appropriate.

The FDA regulates supplements under DSHEA — the Dietary Supplement Health and Education Act of 1994 — but that framework places the burden on manufacturers to self-certify safety before sale. There’s no pre-market approval process for supplements the way there is for pharmaceuticals. That’s precisely why third-party certification carries real weight: it’s the closest thing to independent verification that consumers can reliably access.

What to look for when choosing:

  • NSF Certified for Sport or Informed Sport — both require batch testing for contaminants and label accuracy
  • Creapure® branded creatine — a German-manufactured form with a well-documented purity record, widely used as an industry benchmark
  • A clear statement of ingredient origin — domestic or European manufacturing generally carries fewer quality concerns than unspecified global sourcing

How to Take Creatine Without the Side Effects Most People Complain About

If you’ve had a bad experience with creatine before, it’s worth revisiting before writing it off. A few adjustments address most of the common complaints.

1. Skip the loading phase. Creatine loading — 20g/day for 5–7 days — does saturate your muscles faster, but research shows you reach the same saturation point after about 28 days at 3–5g/day. The only difference is time. If loading gave you GI distress or pronounced water retention, it wasn’t necessary.

2. Take it with food and water. Creatine dissolves imperfectly in cold water (it’s more soluble in warm liquid, for what that’s worth). Pairing it with a meal and staying well above your usual fluid intake — not just on training days — removes the two most common GI complaints.

3. Choose micronized creatine monohydrate. The micronized form has a smaller particle size, which improves solubility and stomach comfort. It’s still standard creatine monohydrate, just better processed. Despite aggressive marketing, neither creatine ethyl ester nor creatine HCl has demonstrated meaningful superiority over monohydrate in controlled trials — and both cost considerably more.

4. Give it four to six weeks before judging it. Intracellular creatine saturation takes time. Many people quit in week two because they haven’t noticed a dramatic change, not realizing they haven’t yet reached the saturation threshold where performance differences become measurable.

The honest reality is that creatine is one of the best-researched, most cost-effective supplements in the category. But that reputation only holds if you’re taking a quality product at an appropriate dose. Most of the side effects people experience are dose-related, quality-related, or both. And the kidney concerns keeping some people wary are, for healthy adults, largely based on a lab value that looks alarming but means something different in the context of creatine supplementation.

Thirty-plus years of research has landed in a pretty consistent place. For healthy people who choose carefully and dose sensibly, the risk profile is genuinely low — and that’s a message worth knowing before you let one number on a lab report make the decision for you.
