Melatonin Side Effects: Why Most Americans Are Taking Far Too Much

A bottle of melatonin at your local pharmacy probably says 5mg or 10mg. The dose that clinical research consistently shows is effective for most adults? About 0.3mg. That’s a 33-fold difference sitting right next to the vitamins, and almost nobody at the checkout counter is going to mention it.

I’ve spent years digging into supplement quality and consumer safety, and melatonin is one of the most misunderstood products in the entire aisle. Consumers assume side effects are rare. They assume bigger doses mean better sleep. And they assume that because melatonin is “natural” — something your own body makes — the risk profile is essentially zero. None of that is fully accurate, and the gap between assumption and reality matters more here than with almost any other supplement category.

Melatonin is a hormone. Not a vitamin, not an herbal extract, not a mineral. A hormone. And the US is one of the only high-income countries in the world where you can buy it off the shelf, in enormous doses, without a prescription. That fact alone should make anyone a little more careful.

What the Research Actually Says About Melatonin Side Effects

The most commonly reported melatonin side effects aren’t dramatic, but they’re consistent across studies:

  • Next-day grogginess (sometimes called the “melatonin hangover”) — particularly common at doses above 1mg
  • Headache, reported in roughly 7–8% of users in controlled clinical trials
  • Dizziness and mild nausea, especially pronounced at higher doses
  • Vivid dreams or nightmares, because your brain is receiving a significant hormonal signal
  • Daytime drowsiness, which becomes a safety concern if you’re driving or operating machinery

These effects are dose-dependent. At 0.5mg, most healthy adults experience minimal side effects. At 10mg — a standard gummy dose — the risk of next-day grogginess climbs substantially. A 2022 meta-analysis published in Sleep Medicine Reviews found that higher melatonin doses were associated with increased morning drowsiness without meaningfully better sleep outcomes compared to lower doses. More melatonin didn’t produce more sleep benefit. It mostly just produced more side effects.

There’s a longer-term concern worth flagging, too. Because melatonin is a hormone — specifically, the hormone your pineal gland releases in response to darkness — repeatedly flooding your system with large supplemental doses may affect your body’s natural production rhythm. The research here is still developing, but several studies have observed measurable suppression of endogenous melatonin in individuals using high-dose supplements over extended periods. It’s not a reason for alarm if you’ve been taking 5mg nightly. But it is a reason to reconsider whether you actually need that dose, and whether a much smaller amount might work just as well.

Children are a separate and more urgent concern. The CDC reported over 52,000 cases of children ingesting melatonin in 2021 — an 87% increase over the previous five years. Poison control centers received tens of thousands of calls. Most of those cases weren’t from children accidentally finding adult bottles; they were from gummy formulations that look and taste like candy. The FDA has not imposed child-resistant packaging requirements specifically for melatonin products, and that regulatory gap has real-world consequences that show up in emergency department data every year.

The Dosing Problem the Industry Isn’t Talking About

Here’s what bothers me most about the melatonin market: the doses being sold bear almost no relationship to what the science supports.

The foundational research on melatonin dosing comes largely from work conducted at MIT, where researcher Richard Wurtman demonstrated that physiological doses in the range of 0.3mg are sufficient to raise blood melatonin levels into the normal nighttime range for most adults. Doses above 1mg push blood levels 10 to 100 times higher than the body naturally achieves at peak nighttime production. And here’s the thing about hormones that the supplement industry tends to gloss over: the dose-response curve isn’t linear. More doesn’t do more. Past a certain threshold, excess melatonin doesn’t make you sleep better — it just stays in your system longer and makes you foggy the next morning.

So why are 5mg and 10mg doses the industry default? A few reasons. Consumers in the US tend to equate higher numbers with stronger products. There’s no competitive advantage to selling a 0.3mg tablet in a market where the guy on the next shelf is advertising “maximum strength 10mg.” And critically, melatonin is regulated in the United States as a dietary supplement under the Dietary Supplement Health and Education Act (DSHEA) of 1994 — which means manufacturers are not required to demonstrate efficacy or standardize dosing before putting a product on shelves.

Compare that to how melatonin is handled almost everywhere else. In the UK, Canada, and most of Europe, melatonin is classified as a prescription medicine. The typical prescription dose in those markets is 2mg in a modified-release formulation for adults over 55 — not 10mg gummies for anyone who wants them. The contrast is stark, and it should tell US consumers something about how little regulatory guardrail exists here.

What Independent Testing Has Found on Melatonin Labels

Label accuracy is a persistent, documented problem in the melatonin category — and the data is harder to dismiss than most people realize.

A study published in JAMA in 2023 analyzed 25 commercially available melatonin gummy products. Actual melatonin content ranged from 74% to 347% of the labeled dose. Nearly half the products contained more than 10% above the stated amount. One product delivered 347% of its labeled amount — meaning a consumer who thought they were taking 5mg was actually ingesting approximately 17mg. That’s not a small variance. That’s a fundamental quality control failure repeated across multiple brands.

This is one area where third-party testing makes a concrete, measurable difference. Products that carry independent verification from organizations like NSF International, USP (United States Pharmacopeia), or Informed Sport have been analyzed in an accredited laboratory to confirm that label claims are accurate within acceptable tolerances. According to testing data from Qalitex Laboratories, melatonin products request label accuracy verification at notably high rates — and the failure rate in that category consistently surprises new clients who assumed their suppliers were trustworthy.

Gummy formulations are particularly prone to variability. Binding melatonin into a sugar-and-gelatin matrix requires even distribution across thousands of individual pieces, and manufacturing variance can cause significant dose differences between gummies in the same bottle. If you care about taking a precise dose of a hormone, a gummy is the least reliable delivery format available.

Five Practical Steps If You’re Currently Taking Melatonin

The goal here isn’t to scare you off melatonin entirely. For jet lag, shift work, and occasional sleep difficulty, the evidence does support its use — at the right dose, timed correctly. These steps will help you use it more safely:

1. Start with 0.5mg or 1mg. Many pharmacies and health food stores now carry low-dose options. Start there. If it’s genuinely ineffective after two consistent weeks, step up incrementally — not directly to 10mg.

2. Time it correctly. Melatonin signals darkness to your brain. Taking it 30–60 minutes before your target bedtime, in a darkened environment, lets it work the way your own physiology would. Taking it too early can shift your circadian rhythm in the wrong direction.

3. Avoid gummies if dosing precision matters. Tablets and capsules from a certified manufacturer distribute the active ingredient far more consistently than gummy matrices. For a hormone where dose matters, that’s worth paying attention to.

4. Look for third-party certification. The USP Verified seal, NSF Certified for Sport mark, or Informed Sport certification all require actual independent laboratory analysis — not just manufacturer self-reporting. If there’s no certification on the label, you’re taking the company’s word for what’s inside.

5. Treat it as a short-term tool. There isn’t strong evidence for long-term nightly use in otherwise healthy adults. If you find you can’t sleep without it after a month or two, the underlying issue — sleep hygiene, stress, screen exposure, room temperature — probably deserves more direct attention than another bottle of supplements.

Melatonin isn’t dangerous in the way that, say, an untested herbal stimulant might be. But it’s also not benign in the way a vitamin C tablet is. It’s a hormone with a measurable physiological effect, sold in doses that mostly reflect marketing logic rather than clinical evidence. Treating it with that understanding is the smarter starting point.
